At Eyes Plus, we go the extra mile before you come in! At the time you call to set your appointment, our helpful staff will ask for your insurance information. Then, a day or two before your appointment, they will call as a courtesy to you, to check your vision and medical coverage eligibility. If, by chance, you are not covered for the exam and/or other services, we will let you know and allow you the choice of coming in regardless of coverage or rescheduling your exam until you are eligible for coverage.
If you have any questions about your insurance before the day of your appointment, we advise you to call your insurance company to receive the most accurate and up-to-date information, as it is subject to change at any time.
IMPORTANT NOTIFICATION: All insurance information (coverage, co-pays, co-insurance, etc.) is ADVISORY ONLY and is subject to change PER YOUR INSURANCE COMPANY. It is ultimately the patient's responsibility to know their insurance details (coverage, co-pays, co-insurance, etc.) and to pay for any balances their insurance company charges back to them via our office as we are filing for services rendered and products purchased on the patient's behalf. We are not responsible for any changes that the insurance company makes regarding the patient's coverage, co-pays, etc. from the time the patient is seen to the time the patient's insurance company reimburses our office. All filings and billings are done through an external insurance processing agency, Physician's Billing Solutions, If you have any questions regarding a bill received from Physician's Billing Solutions, feel free to call the number at the bottom of the statement.
When you choose Eyes Plus to be the source of your eye care service, we do this as an added courtesy service to you.
