We do apologize for any confusion or inconvenience this may cause. Please contact your individual insurance’s CUSTOMER SERVICE for accurate Provider information.
At Eyes Plus, we go the extra mile even "before" you come in! At the time you call to set your appointment, our helpful staff will ask for your insurance information. Then, a day or two before your appointment, as a courtesy to you, they will call your insurance company to check your vision and medical coverage eligibility. If, by chance, you are not covered for the exam and/or other services, we will let you know and allow you the choice of coming in regardless of coverage or rescheduling your exam until you are eligible for coverage.
If you have any questions about your insurance before the day of your appointment, we advise you to call your insurance company directly to receive the most accurate and up-to-date information, as it is subject to change at any time.
IMPORTANT DISCLAIMER NOTIFICATION: All insurance information (coverage, co-pays, co-insurance, etc.) is ADVISORY ONLY and is subject to change PER YOUR INSURANCE COMPANY.
